New Patient

Julie Ann Floyd, MD is currently accepting new patients.

If you are interested in meeting with Dr. Floyd, you must first schedule an appointment with us. Scheduling is easy- please send an email to Appointments@TropicalWellness.net or call 305-292-4970.

New patients need to bring the following items to the first appointment:

  • Insurance cards
  • Government issued photo ID
  • List of current medications

To expedite your paperwork, please print and fill out the New Patient Information form and bring it with you to your first appointment.
