FAA Medicals

Julie Ann Floyd, MD is an appointed Senior FAA Aviation Medical Examiner. Dr. Floyd provides Federal Aviation Administration (FAA) flight examinations for 1st, 2nd, and 3rd classes. To schedule a flight examination, please conatct us at Appointments@TropicalWellness.net or call (305) 292-4970.

Dr. Floyd is also a commercial pilot who places a priority on scheduling and completing your FAA medical certification process on the timeline you need. We offer expanded office hours which often helps speed the process of obtaining your medical certification.

Before your FAA Physical, you will need to go to the FAA website at FAA.gov. You will be required to sign in with a username and password. After successfully logging in, you will need to click on the "Medical Certification" link. This link will then take you directly to the required medical form. After completion, please be sure to click on the "Submit" button on the form page. This will ensure that the FAA receives the required physical forms. Within a few hours the FAA will send you an email stating that they have recieved your completed medical forms. After reviewing the email, go back to FAA.gov and click on the 8500 form. When asked, choose to open it as a PDF. Once the PDF is opened, print it out and bring it with your on your next visit, along with your pilot's license, driver's license, and old medical certificate.
